GRADING SYSTEM FOR WOUNDS

There are two accepted methods for wound classification including the: Wagner Grading System and the Texas Classification system.


The Wagner Grading System for Diabetic Feet consists of 6 grades including:

0 - Intact Skin

1 - Superficial ulcer of skin or subcutaneous tissue

2 - Ulcers extending into tendon, bone or capsule

3 - Deep ulcer with osteomyelitis or absess

4 - Gangrene of toes or forefoot

5 - Gangrene of midfoot or hindfoot 


The Texas Classification also incorporates infection and ischemia which provides more details about the wound.


Subcategorized into 4 sections and 3 grades:

A - areas of pressure (preulcerative)

B - infection

C - ischemia

D - infection + ischemia


3 Grades:

0 - Preulcerative lesions

1 - Superficial ulcer not including tendon, capsula or bone

2 - Deep ulcer including tendon, capsule but not bone

3 - Deep ulcer including bone and articulation
