SURGICAL MANAGEMENT OF ANKLE FRACTURES IN PATIENTS WITH DIABETES

Ankle fractures are normally straight forward however in patients with diabetes this can lead to bad outcomes. Patients with diabetes have a higher risk of complications after an ankle fracture including: wound infection, Charcot neuropathy, amputation etc.


Ankle fractures can be treated non-operatively/conservatively with a cast or brace however, conservative treatment can sometimes have risks as well, especially in the diabetic population. Casts can sometimes leave pressure points leading to diabetic ulcerations or infections. Casts can sometimes hinder our view of the blood flow of the foot which can lead to other complications as well. Patients with diabetes more often that not have other health conditions as well including: peripheral vascular disease, neuropathy and poor medical compliance. Therefore, patients who are treated conservatively should be followed up with closely to prevent any new ulcerations or infections as they could sometimes be limb threatening.
